Ambulatory phlebectomy is an outpatient procedure developed by dermatologic surgeons that removes superficial veins through small, slit-like incisions in the skin.

It is the surgical removal of the gall bladder. 

About Procedure – Detailed Information: 

Indications:  

Symptomatic gallstones 

Acute, Chronic – Cholecystitis 

Acalculus Cholecystitis 

Empyema gallbladder 

Muscele gallbladder 

Approach: 

Right subcostal incision (Kocher’s) 

Right Paramedian 

Horizontal Incision 

Mayo Robson Incision 

Laparoscopic Approach – Procedure: 

Position: Supine position, head end up & right side up 

Anesthesia: General Anesthesia 

Ports: 10 mm port in the umbilicus, 10mm port in middle epigastrium 

Two 5mm port at mid clariculase and auterior axillary line in subcostal region 

Steps:  

Under general anesthesia 

Pt. is in the supine position 

Parts painted and draped 

10mm supraumbilical part (Camera) port created by open technique 

Pneumoperitoneum created 

12 mm epigastric part and two 5mm instrumentation ports taken 

Cystic duct dilated 

Adhesiolysis done 

Calot’s triangle identified and dissected 

Cystic artery identified, clipped with hemoclip, and cut 

Cystic duct identified, clipped with 3 hemoclips & cut 

Cholecystectomy done 

Gallbladder extracted through a 12 mm epigastric port 

28 No. Abdominal drain kept in gall bladder fossa 

Haemostaris confirmed & ports closed using port Viveyl 

Skin closure done 

Dressing done 

Gall bladder sent for HPE

About Procedure: Detail Information:

This is a newer popular, ideal method of appendicectomy. It has become the gold standard method of treatment.

Indication:

· All Kind of Appendicitis, including appendicular abscess

· Suitable for obese patients & females of reproductive age

Procedure:

· Under General Anaesthesia

· supine portion given

· parts painted & draped

ports placements

· 10 mm = Camera port at Umbilicus

· Two 5 mm = one on each side of the lower abdomen or one on the left side & another on, the lower midline

10 mm = Working port

· Pneumsperitrneum is created using CO2

· Appendix is held with a grasper

· Merrappendix is Cauterised by bipolar Cautery

· Base of the Appendix is ligated with loop ligature

· Intracorporeal ligature placed using viceyl 2-0 suteeve material

· Appendix is removed through 20mm working port along with reducer; Haemostan’s confirmed

· Umbilical port is closed in two layers by port vicryl

· Other ports closure done

· Dressing done

Specialization of the Procedure:

Position- Head down position with right tilt is needed in Supine

Anaesthesia- General Anaesthesia
